چکیده

Objectives To evaluate the accuracy of a new COVID-19 prognostic score based on lung ultrasound (LUS) and previously validated variables in predicting critical illness. Methods We conducted single-center retrospective cohort development internal validation study Worsening Score (COWS), combination COVID-GRAM (GRAM) LUS. Adult patients admitted to emergency department (ED) were enrolled. Ten identified by GRAM, days from symptom onset, LUS findings, peripheral oxygen saturation/fraction inspired (P/F) ratio analyzed. as single predictor was assessed. evaluated GRAM model's performance, impact adding LUS, then developed model most predictive variables. Results Among 274 enrolled, 174 The 51 at high risk developing illness 132 low risk. over 15 (range 0 36) associated with higher (RR, 2.05; 95% confidence interval [CI], 1.52-2.77; area under curve [AUC], 0.63; CI 0.676-0.634). newly relies five classify high- low-risk an overall 80% negative value 93% (95% CI, 87%-98%). Patients scoring more than 0.183 COWS showed RR 8.07 4.97-11.1). Conclusions accurately identify who are unlikely need intensive care unit (ICU) admission, preserving resources for remaining high-risk patients.

Multicentre validation of the bedside paediatric early warning system score: a severity of illness score to detect evolving critical illness in hospitalised children

INTRODUCTION The timely provision of critical care to hospitalised patients at risk for cardiopulmonary arrest is contingent upon identification and referral by frontline providers. Current approaches require improvement. In a single-centre study, we developed the Bedside Paediatric Early Warning System (Bedside PEWS) score to identify patients at risk. The objective of this study was to valida...

External validation of a prehospital risk score for critical illness

BACKGROUND Identification of critically ill patients during prehospital care could facilitate early treatment and aid in the regionalization of critical care. Tools to consistently identify those in the field with or at higher risk of developing critical illness do not exist. We sought to validate a prehospital critical illness risk score that uses objective clinical variables in a contemporary...

Bench-to-bedside review: Chloride in critical illness

Chloride is the principal anion in the extracellular fluid and is the second main contributor to plasma tonicity. Its concentration is frequently abnormal in intensive care unit patients, often as a consequence of fluid therapy. Yet chloride has received less attention than any other ion in the critical care literature. New insights into its physiological roles have emerged together with progre...

Bedside Tool for Predicting the Risk of Postoperative Atrial Fibrillation After Cardiac Surgery: The POAF Score

BACKGROUND Atrial fibrillation (AF) remains the most common complication after cardiac surgery. The present study aim was to derive an effective bedside tool to predict postoperative AF and its related complications. METHODS AND RESULTS Data of 17 262 patients undergoing adult cardiac surgery were retrieved at 3 European university hospitals. A risk score for postoperative AF (POAF score) was...

The Critical-Care Pain Observation Tool: A useful tool for pain assessment in intensive care units

Pain is a major concern in all intensive care units (ICUs). The proper assessment and management of pain is one of the main goals of patient care in ICUs. Improper evaluation of pain and its over or under treatment can cause significant problems in the process of patient management in ICU. Since most ICU patients are unable to communicate correctly with the ICU team and explain their level of p...
